Cape Fear Healthy Opportunities Pilot

The Healthy Opportunities Pilot (HOP) is an innovative new approach to test and evaluate non-medical interventions related to housing, food, transportation and interpersonal safety and toxic stress to eligible Medicaid enrollees. This Medicaid pilot program was developed by North Carolina Department of Health and Human Services (NCDHHS) with a goal to improve health and reduce health care costs by investing in evidence-based interventions.

Care Management for At-Risk Children (CMARC)

The Medicaid program currently offers a set of care management services for at-risk children ages zero-to-five. The program coordinates services between healthcare providers, community programs and supports, and family support programs. Care Management for At-Risk Children (CMARC) is provided by the local health departments.

Care Management for High-Risk Pregnancies (CMHRP)

CMHRP is a North Carolina Medicaid program that provides free care management services for women at risk for a poor birth outcome. CMHRP Services are initiated based on information obtained from the standardized screening tool administered to all pregnant Medicaid and Medicaid eligible recipients during their first prenatal visit. Other referral options include self-referral, risk stratification, or direct referrals from providers or community agencies such as WIC or DSS.
