Welcome to the North Carolina Reproductive Health & Wellness Resource Hub

This site seeks to strengthen transitions and continuity of care across North Carolina by curating a knowledge exchange platform. With the hope of organizing our resources at the state level, a multi-disciplinary team of champions has joined forces to highlight what resources exist in their regions and what has helped them bridge the gap between surviving and thriving.

This project was made possible thanks to guidance and support from the Maternal Health Learning and Innovation Center (MHLIC). You may notice that the structure of our site is inspired by their own Maternal Health Repository. We use their best practices in regards to formatting with permission and gratitude. Special thanks to our main contributors, the Perinatal Nurse, Family Medicine, Pediatric, and Ob/Gyn Champions representing all six Maternal health regions of North Carolina.

Purpose

Established in early 2020, the Maternal Health Innovations Program aims to improve the quality of care delivered by outpatient providers caring for birthing people in the pregnant, postpartum, and interconception periods. Our resource hub intends to house a wide range of innovative programs, policies, templates, screening/assessment tools, etc. The purpose of this site is to elevate successful strategies that guide coordination and continuity of care to provide whole-person care.

The content was collected by an interdisciplinary team of healthcare professionals striving for accuracy and relevance. This site was created to meet the need for increasing workforce capacity.
