New Patient Referrals

Psychiatry and Medication Management

We are pleased to share that we are now accepting referrals for Psychiatric Evaluation and Medication Management for those 17 and under. If you are an established MAHEC patient, we encourage you to speak with your MAHEC provider about a referral to the Psychiatry office. If you are not a MAHEC patient, you can self-refer using this referral form.

Therapy Services

We are accepting referrals for Therapy Services for all ages, but only with a referral from a MAHEC provider. We hope to be open to outside referrals soon and appreciate your patience as we continue working to expand our team and improve access to care.

School Based Therapy Referrals

Please click here to fill out our online form if you are referring a child to our School Based Therapy program. After referring the patient, please click here to complete our School Based Therapy Welcome Packet.

For mental health and medical emergencies, you should call 911 or reach out to Mobile Crisis Management at 888-573-1006.
