Shadowing Affiliate Onboarding Documents

The items below are intended for use by MAHEC students who are shadowing for a period of less than two weeks. Shadowing affiliates will be considered in order of request and must be approved by the Center for Health Professions Education. All steps below must be completed prior to the shadowing start date.

Please complete items 1-4 shown below. Item 5 is only required if Affiliate is under the age of 18. Once completed, submit all items to your MAHEC Host Representative directly.

1. MAHEC Shadowing Affiliate Application Packet: All forms must be fully signed and completed in order to be accepted.

2. COVID-19 Vaccine and Influenza Vaccines: Documentation of COVID-19 and Influenza vaccines must be submitted to your MAHEC Host Representative prior to your first day of affiliation. The MAHEC Vaccination Program Policy allows limited medical and religious exemptions at this time for these two requirements.

3. COVID-19 Screening Form: Please defer to your student onboarding Manager. 4. Student, Intern and Affiliate Training: Review the training slides below on Compliance, HIPAA, Research, Risk, and OSHA Safety. Sign the attestation form in the Application Packet confirming that you have completed this orientation. 5. MAHEC Minor Consent and Release Form: (Required only if under age 18)

Submit all forms to your MAHEC Affiliate Host. You may contact Talent Management at AskTalent@mahec.net if you do not have contact information for your Affiliate Host.
