Affiliate Worker Onboarding Documents

The items below are intended for use by any contingent/contract worker or vendor seeking affiliation with MAHEC. All onboarding steps below must be completed prior to the first day of affiliation.

Please complete items 1-4 shown below. Item 5 is only required if Affiliate is under the age of 18. Once completed, submit all items to your MAHEC Host Representative directly.

1. Affiliate Worker Application Packet: Upon acceptance of your application, you will receive a background screening request from our provider "CRC" Criminal Record Check. Once completed, your MAHEC host will be notified. All forms must be fully signed and filled out to be accepted. [click to download]

2. COVID-19 Vaccine and Influenza Vaccines: Documentation of COVID-19 and Influenza vaccines must be submitted to your MAHEC Host Representative prior to your first day of affiliation. The MAHEC Vaccination Program Policy allows limited medical and religious exemptions at this time for these two requirements.

3. Student, Intern, and Affiliate Training: You will receive an email from Smartsheet containing a link to mandatory training modules as well as policies that need to be completed and reviewed. Once finished, you will fill out and submit an attestation form via the same link. 4. MAHEC Minor Consent and Release Form: Required only if under age 18. [click to download]

NOTE: Certain Affiliation Agreements require laboratory performed drug-screened. A member of the MAHEC Talent Management team will contact you with further information if drug-screening is required for your affiliation.
