NC DHHS has developed partnerships with North Carolina Area Health Education Centers (NC AHEC) to provide practice support, technical assistance, and continuing professional development to help Tailored Care Management providers who have applied and passed the desk review in the transition to Tailored Care Management. The TCM Continuing Professional Development programs are designed to assist TCM care managers, supervisors, and extenders with evidence-based practices in all treatment settings for the successful implementation of TCM.
This training is designed to provide an overview of the Transitions to Community Living (TCL) program, and key initiatives for providers planning to provide TCL. This program is the first of two trainings required of North Carolina providers for the TCL distinction to administer TCL services in NC. This is not a required course to provide Tailored Care Management.
Approximate time to complete the course: 2 hours
Learner content includes: Videos and a corresponding course handout
