The North Carolina Department of Health and Human Services (NC DHHS), Department of Health Benefits (DHB) has developed a partnership with North Carolina Area Health Education Center (NC AHEC) to provide practice support, technical assistance, and continuing professional development to help Tailored Care Management (TCM) providers who have applied and passed the desk review to transition into the TCM service.
The TCM Learning Collaborative is designed to assist TCM candidates in developing the administrative tools to implement best practices within all treatment settings and among all providers. These sessions will be led by NC AHEC Practice Support to facilitate a coordinated learning experience with access to subject matter experts across the state. The Year 3 Learning Collaborative is focused towards Round 3 and future rounds of TCM implementation on the first week of each month, and focuses on NC DHHS, DHB TCM Office Hours on the third week of each month.
This collaborative includes 1-hour live webinar sessions occurring twice a month.
Please note: Participants must attend the entire session to receive credit. No credit is provided for DHB Office Hours. All sessions will be recorded and posted for future access on the TCM Web Portal. All TCM courses are for TCM certified AMH+/CMA employees only; contracted CINs and other agencies contracted to provide care management do not receive these courses for free. A discount code to allow free access will be provided to your agency administrator by your NC AHEC practice support coach. Please input your agency information at checkout. By registering for this program, participants understand that their contact information may be shared with NC DHHS and the BH/IDD Tailored Plans (LME/MCOs). By attending this event, participants agree to audio and video recording of the session, and posting of the recordings in an LMS.

Administrators and supervisors from advanced medical home+ (AMH+) and care management agencies (CMA) candidates who have applied and passed the desk review for Tailored Care Management, including physicians, nurses and various behavioral health substance use professionals
TCM providers will learn how to:

Identify fundamentals of the TCM services, including the 1915i and HCBS, comprehensive care assessments, and patient panels/risk lists |

Identify best practices from members with complex diagnoses and how to incorporate community inclusion |

Improve communication through feedback sessions with DHB during TCM Office Hours (no credit provided for these sessions) |

$960.00MAHEC has a pay-up-front policy for all CE programs. The only exceptions will be for pre-approved programs where an individual payment plan is appropriate. Registrations received without accompanying payment will not be processed and participants who have not paid the course fee will not be admitted into the program.
