NC DHHS has developed partnerships with North Carolina Area Health Education Center (NC AHEC) to provide practice support, technical assistance, and continuing professional development to help Tailored Care Management providers who have applied and passed the desk review in the transition to Tailored Care Management. The TCM Continuing Professional Development programs are designed to assist TCM care managers, supervisors, and extenders with evidence-based practices in all treatment settings for the successful implementation of TCM.

This series consists of 13 à la carte elective courses of approximately one hour each that can be used to assist TCM staff working in Tailored Care Management. Care manager supervisors may assign care managers and care manager extenders to use these trainings in order to assist in further skill development or knowledge gaps within TCM service provision and supervision.

Sessions may be selected individually, but please ensure that all desired courses are selected prior to checking out, as these choices can only be made once each year per learner.

Credit accumulation is based on the total hours selected. A minimum of one full hour of credit must be selected in order to receive credit.

Target Audience

TCM care managers, supervisors, and extenders from the Tailored Plans, TCM certified clinically integrated networks (CINs), TCM certified advanced medical homes+ (AMH+) and care management agencies (CMA)

Objectives

Participants will be able to:

  • Describe best practices for TCM care managers working with the following populations: traumatic brain injuries (TBI), individuals with trauma, individuals with orthopedic impairments, perinatal/child/ adult behavioral health and unmet resource needs.
  • Demonstrate examples of providing TCM care management for individuals needing: tobacco cessation, benefits acquisition, mutual aid resources, and employment.
  • Demonstrate and review best practices in care management in the subjects taken within this refresher course series.
