NC DHHS has developed partnerships with North Carolina Area Health Education Center (NC AHEC) to provide practice support, technical assistance, and continuing professional development to help Tailored Care Management providers who have applied and passed the desk review in the transition to Tailored Care Management. The TCM Continuing Professional Development programs are designed to assist TCM care managers, supervisors, and extenders with evidence-based practices in all treatment settings for the successful implementation of TCM.

This system of care training outlines the core values of system of care, provides guidance in making connections to improve SOC partnerships within the community, and discusses the key role family and youth play in NC's System of Care and specific ways the family/youth voice should be incorporated when developing treatment plans, family-centered practices, policies, and activities to support children, youth, and families.

Target Audience

TCM care managers, supervisors, and extenders from the Tailored Plans, TCM certified advanced medical homes+ (AMH+) and care management agencies (CMA)

Objectives

Participants will be able to:

  • Describe an overview and understanding of what is meant by the term "System of Care," its core principles, and how it is to be implemented in North Carolina
  • Explain the principles of system theory and leading models of System of Care implementation
  • Give examples of 3-5 ways to apply System of Care principles to support children, youth, and families in their communities and schools
  • Demonstrate how to implement the System of Care principles and apply specific methods to provide care for NC youth and families receiving services as part of North Carolina's Tailored and Standard Plans
