NC DHHS, Department of Health Benefits (DHB) has partnered with NC AHEC to assist in the implementation of Tailored Care Management, which was launched on December 1, 2022. NC AHEC provides practice support, technical assistance, and continuing professional development to help Tailored Care Management providers who have applied and passed the desk review in the transition and continuous quality improvement of Tailored Care Management. TCM courses are designed to assist TCM providers in implementing best practices within all treatment settings and among all providers.
This presentation will equip Tailored Care Managers with the knowledge needed to be effective care managers in collaborating with DSS Child Welfare Workers to promote positive health outcomes for children served by the child welfare system.
