The NC DHHS Division of Health Benefits (DHB) has partnered with NC AHEC to assist in the implementation of Tailored Care Management which was launched December 1, 2022. This course is Part 2 of the 26 required sessions. This material has been approved and is required by DHHS/DHB to begin TCM services with members. DHB is requiring that all care managers, supervising care managers, and care manager extenders complete the TCM Core Curriculum Training Bundle: Part 2 within six (6) months of hire.
This on-demand course contains the following 22 required courses:
1. TCM Role with Members Involved in the Criminal Justice System (2 hours)
2. TCM Role with Members with I/DD (1.75 hours)
3. TCM Role with Members with Orthopedic Impairments (2.50 hours)
4. Critical Incident Response and Debriefing (3 hours)
5. Breathe Easy NC: Becoming Tobacco-Free Training Series - Standard-of-Care Tobacco Use Treatment: Behavioral Interventions and Medications (1.50 hours)
6. Breathe Easy NC: Becoming Tobacco-Free Training Series - Protecting Youth from Secondhand Smoke, E-cigarettes, and Helping Them Quit (1 hour)
7. Incorporating Trauma-Informed Care in the TCM Model (2.75 hours)
8. Employment Assistance for Individuals in Tailored Care Management (2 hours)
9. Motivational Interviewing Toward Health Behavioral Change (2.25 hours)
10. Transitions to Community Living (2.25 hours)
11. Social Determinants of Health: Practical Strategies for Assessment and Response (2.50 hours)
12. TCM Person Centered Thinking for Healthcare Professionals (2.75 hours)
13. Legal Rights and Services Used to Support TCM Members (2.75 hours)
14. Increasing Independence through Assistive Technologies (2.50 hours)

16. Telehealth in the World of Tailored Care Management (1.50 hours)

Target Audience
Tailored Plans, AMH+ and CMA agencies certified to provide Tailored Care Management. Audience should include TCM supervisors, care managers and extenders.
Objectives
As part of the TCM Required Curriculum participants will be able to:
- Describe best practices for Care Managers working with the following populations: criminal justice, I/DD, orthopedic impairments, perinatal and child and adult substance use recovery
- Demonstrate examples of providing Care Management for individuals needing: tobacco cessation, full range of transitional services, benefit acquisition, self help/mutual aide, assessment and response, employment assistance
- Develop skills specifically for TCM in: motivational interviewing, critical incident response, trauma informed care, legal rights, cultural/linguistic competencies, member and staff ethics and safety
- Develop a working competency of telehealth services and resources to provide team based care for members, person centered thinking for healthcare professionals, and independence through adaptive technology
