Tailored Care Management Core Curriculum Bundle: Part 1

The NC DHHS Division of Health Benefits (DHB) has partnered with NC AHEC to assist in the implementation of Tailored Care Management which was launched December 1, 2022. This course is Part 1 of the 26 required sessions. This material has been approved and is required by DHHS/DHB to begin TCM services with members. The AMH+ or CMA must ensure that all TCM care managers, care manager extenders, and care manager supervisors complete training on the following core modules before being deployed to serve TCM members.

This on-demand course contains the following four required courses:

1. NC Innovations Waiver and Home and Community-Based Services (1.75 hours)
2. Team-Based Care in TCM (2.50 hours)
3. The Intersection of Chronic Disease and Comorbid Behavioral Health Management: An Application of the SBIRT Model in Primary Care (3.50 hours)
4. Providing Whole-Person Care for Individuals with TBI and their Families (3 hours)

Approximate time to complete the course: 10.75 hours
Learner content includes: A video and corresponding course handout for each module

Individuals must complete TCM Core Curriculum Bundle: Part 1 before being deployed to serve members.
Individuals must complete TCM Core Curriculum Bundle: Part 2 within 6 months of being deployed to serve members.

One of the final lessons of Core Curriculum Bundle: Part 1 is a link and code to register for Core Curriculum Bundle: Part 2.

Target Audience

Tailored Plans, AMH+ and CMA agencies certified to provide Tailored Care Management. Audience should include TCM supervisors, care managers and extenders.

Objectives

As part of the TCM Required Curriculum participants will be able to:

• Describe the health home model, Innovations Waiver, and home and community based services and eligibility criteria.
• Provide an overview of the TCM Care Manager and team members in integrated care systems and team-based care, their best practices, and strategies for working with interdisciplinary teams.
• Provide an overview of the common physical and behavioral health comorbidities associated with BH I/DD Tailored Plan populations with examples of interventions for screening, assessment, and management of these co-occurring disorders.
• Provide an overview of important concepts in Traumatic Brain Injury (TBI), including diagnostic criteria, clinical presentations and treatments, impact on community living and health, and impact on caregivers and family.
