July 1st, 2021 marked the start of NC Medicaid Transformation with the transition to Standard Care Plans. NC DHHS has begun the process of preparing for Tailored Care Management (TCM) to begin on December 1st, 2022. To prepare for this, DHHS has developed partnerships with North Carolina Area Health Education Center (NC AHEC) to provide practice support, technical assistance, and continuing professional development to help Tailored Care Management providers who have applied and passed the desk review in the transition to Tailored Care Management. The TCM Continuing Professional Development programs are designed to assist TCM candidates with evidenced-based practices in all treatment settings for the successful implementation of TCM.

The integration of behavioral healthcare into primary care is now widely considered an effective strategy for improving outcomes for patients with physical and behavioral health conditions. Reverse integration of primary care into behavioral healthcare is another form of integration that produces an improved quality of care and improved patient outcomes.


This workshop will begin with an overview of the reverse integration process and how to implement this new model of care. Secondly, we will focus on comprehensive, evidence-based primary care patient intake; this section will include best practices for measuring vital signs, exam room setup considerations, and point-of-care laboratory testing for behavioral health practitioners considering integrating primary care services - all of these topics are covered through a patient-centered lens. In the third segment of this workshop, we will review key concepts and strategies for building and optimizing integrated health care programs with sustainability as the goal.

Audience

Administrators and supervisors from advanced medical home+ (AMH+) and care management agencies (CMA) candidates who have applied and passed the desk review for Tailored Care Management, including physicians, nurses and various behavioral health substance use professionals

Objectives

As part of the TCM Continuing Professional Development series, participants will learn how to:

check Identify physical health needs in behavioral health care
check Compare patient intake data necessary for behavioral health intake with that needed for primary care visits
check Identify functionality necessary to have within your selected electronic health record to accommodate integration of behavioral health and primary care patient needs
check Appraise and identify tools for effective financial planning and billing in integrated care
check Describe how return on investment calculations provide a financial metric of profitability comparison for making sound financial decisions
check Describe integrated behavioral health approaches to care
check Choose appropriate language and setting (through a lens of health equity) to elicit information from patients to complete a primary care visit intake
check Describe basic exam room setup and supplies, point of care labs and other services used in primary care
check Identify specific simple formulas to design cost savings and direct revenue return on investment calculations

Cancellation Policy

MAHEC has a pay-up-front policy for all CE programs. The only exceptions will be for pre-approved programs where an individual payment plan is appropriate. Registrations received without accompanying payment will not be processed and participants who have not paid the course fee will not be admitted into the program.

Unless otherwise noted in course materials, the following cancellation policy applies to all programs:

  • Cancellations must be in writing (fax, email, or mail).
  • Cancellations received more than 2 weeks prior to the event will receive 100% refund.
  • Cancellations received between two weeks and two full business days prior to the first day of the event are refunded at 70% of the registration fee subject to a minimum $25 cancellation fee.
  • No refunds or credits will be given for cancellations received less than two full business days prior to the event.
  • No vouchers will be issued in lieu of a refund.
