This course dives into the crucial aspects of medication management, focusing on reconciliation, monitoring, and patient adherence within the context of TCM. Participants will gain practical skills and knowledge essential for optimizing patient care, ensuring medication safety, and improving health outcomes. Participants will explore effective strategies for reconciling medications during key transitions, as well as best practices for provider medication monitoring to improve patient adherence and health outcomes.
