Develop awareness annually of: assistive technologies, chronic disease/comorbid BH treatment, critical Incident response debriefing, transitions to community living, social determinants of health, person centered thinking, overviews/updates on the full array of Medicaid services and eligibility criteria, transitional care services and best practices in child and family treatment teams/system of care. |

Demonstrate examples of providing TCM care management for individuals needing: full range of transitional services, critical incident response debriefing, care management assessment and response, and assistive/adaptive technologies. |
