NC DHHS has developed partnerships with North Carolina Area Health Education Centers (NC AHEC) to provide practice support, technical assistance, and continuing professional development to help Tailored Care Management providers who have applied and passed the desk review in the transition to Tailored Care Management. The TCM Continuing Professional Development programs are designed to assist TCM care managers, supervisors, and extenders with evidence-based practices in all treatment settings for the successful implementation of TCM.

This bundle of courses is a set of 11 one-hour refresher courses required for Tailored Care Management service provision. Care managers, care manager extenders, and supervisors have a full year to complete this training following the end of their second full year of deployment (see the TCM Provider Manual Appendix, p.87).

This bundle includes:

Audience

TCM care managers, supervisors, and extenders from the Tailored Plans, TCM certified clinically integrated networks (CINs), TCM certified advanced medical homes+ (AMH+) and care management agencies (CMA)

Objectives

check Describe best practices for TCM care managers working with the following populations: criminal justice, I/DD, perinatal, and child and adult behavioral health.
check Develop awareness annually of: assistive technologies, chronic disease/comorbid BH treatment, critical Incident response debriefing, transitions to community living, social determinants of health, person centered thinking, overviews/updates on the full array of Medicaid services and eligibility criteria, transitional care services and best practices in child and family treatment teams/system of care.
check Demonstrate examples of providing TCM care management for individuals needing: full range of transitional services, critical incident response debriefing, care management assessment and response, and assistive/adaptive technologies.

Cancellation Policy

MAHEC has a pay-up-front policy for all CE programs. The only exceptions will be for pre-approved programs where an individual payment plan is appropriate. Registrations received without accompanying payment will not be processed and participants who have not paid the course fee will not be admitted into the program.

Unless otherwise noted in course materials, the following cancellation policy applies to all programs:

  • Cancellations must be in writing (fax, email, or mail).
  • Cancellations received more than 2 weeks prior to the event will receive 100% refund.
  • Cancellations received between two weeks and two full business days prior to the first day of the event are refunded at 70% of the registration fee subject to a minimum $25 cancellation fee.
  • No refunds or credits will be given for cancellations received less than two full business days prior to the event.
  • No vouchers will be issued in lieu of a refund.
  • Transfers/substitute(s) are welcome (please notify us in advance of the program).
