NC DHHS has developed partnerships with North Carolina Area Health Education Centers (NC AHEC) to provide practice support, technical assistance, and continuing professional development to help Tailored Care Management providers who have applied and passed the desk review in the transition to Tailored Care Management. The TCM Continuing Professional Development programs are designed to assist TCM care managers, supervisors, and extenders with evidence-based practices in all treatment settings for the successful implementation of TCM.

Many care managers have limited experience working with healthcare providers. Likewise, many medical providers have limited experience working with care managers. This course aims to close this divide and present best practices in engagement, health, and behavioral health coordination; it will cover the tools and skills needed when working on health outcomes, working in teams, communicating between providers, and how to best support the patient's/client's medical and behavioral health goals.

Target Audience

TCM care managers, supervisors, and extenders from the Tailored Plans, TCM certified clinically integrated networks (CINs), TCM certified advanced medical homes+ (AMH+) and care management agencies (CMA)

2.500 NACCM

The National Academy of Certified Care Managers (NACCM) certifies that continuing education training provided by a NC Area Health Education Center shall be approved for no more than 15 contact hours for any given Care Manager Certified (CMC) recertification period.
