NC DHHS, Division of Health Benefits (DHB) has partnered with NC AHEC to assist in the implementation of Tailored Care Management (TCM), which was launched December 1, 2022. NC AHEC provides practice support, technical assistance, and continuing professional development to help Tailored Care Management providers who have applied and passed the desk review in the transition and continuous quality improvement of Tailored Care Management. The TCM Learning Collaborative is designed to assist TCM providers in developing the administrative tools to implement best practices within all treatment settings and among all providers. The Collaborative will be led by NC AHEC Practice Support to facilitate a coordinated learning experience with access to subject matter experts across the state.

This session will discuss signs and symptoms of complex health needs that can help guide your approach to comprehensive care management assessment, care planning, and individualized service plans.


This course is not intended for sale. It is for certified AMH+/CMA providers and Tailored Plans only. A discount code to allow free access will be provided to you by your agency administrator. CINs and other agencies providing comprehensive care management that do not have a contract with MAHEC do not receive these courses for free. To contract with MAHEC, please contact Scott Melton.

Target Audience

TCM care managers, supervisors, and extenders from the Tailored Plans, TCM certified advanced medical homes+ (AMH+) and care management agencies (CMA)

Objectives

Participants will learn how to:

  • Triage and refer complex needs for MH, SU, IDD, TBI individuals
  • Identify ways to get individuals with complex needs in treatment, quickly while prioritizing vital needs
  • Utilize the SBIRT model as method of prioritizing the most important needs of individuals with complex health needs
