Description
- Once registration has closed on July 15 -- if you are already registered for individual TCM course options -- you will have 90 days (October 14) to complete the course(s) before the courses are permanently closed.
- Any participants who do not complete the individual courses by October 14 will have to take Bundle 1 and Bundle 2 to complete their TCM training.
- As of July 15th, All new hires and hires returning to the field will have to take the bundled courses even if they are only missing 1 or 2 courses

July 1st, 2021 marked the start of NC Medicaid Transformation with the transition to Standard Care Plans. NC DHHS has begun the process of preparing for Tailored Care Management (TCM) to begin on December 1st, 2022. To prepare for this, DHHS has developed partnerships with North Carolina Area Health Education Center (NC AHEC) to provide practice support, technical assistance, and continuing professional development to help Tailored Care Management providers who have applied and passed the desk review in the transition to Tailored Care Management. The TCM Continuing Professional Development progams are designed to assist TCM candidates with evidenced-based practices in all treatment settings for the successful implementation of TCM.
This course will train healthcare workers in Person Centered Thinking, which will help in developing service plans that work collaboratively with their behavioral health goals; it will also briefly provide motivational interviewing techniques that help in collaborating with patients to establish healthcare goals.
Approximate time to complete the course: 2.50 hours

Participants have unlimited time to complete this course and access course materials.

Target Audience
Administrators and supervisors from advanced medical home+ (AMH+) and care management agencies (CMA) candidates who have applied and passed the desk review for Tailored Care Management, including physicians, nurses and various behavioral health substance use professionals
Objectives
As part of the TCM Continuing Professional Development series, participants will:
- Identify the differences in "Important To" and "Important For"
- Identify the components of the BioPsychoSocial model
- Understand the concepts of fixing vs supporting someone as they relate to Person Centered Thinking (PCT) & Planning through discussion
- Become aware of how culture influences us
- Become familiar with the 11 PCT Skills and how they can be used through discussing and practicing 1 or 2 of those 11 skills
- Identify one way MI concepts enhance Person Centered Thinking & Planning
- Identify the 5 concepts in developing goals that are successful
