July 1st, 2021 marked the start of NC Medicaid Transformation with the transition to Standard Care Plans. NC DHHS has begun the process of preparing for Tailored Care Management (TCM) to begin on December 1, 2022. To prepare for this, DHHS has developed partnerships with North Carolina Area Health Education Center (NC AHEC) to provide practice support, technical assistance, and continuing professional development to help Tailored Care Management providers who have applied and passed the desk review in the transition to Tailored Care Management. The TCM Continuing Professional Development programs are designed to assist TCM candidates with evidenced-based practices in all treatment settings for the successful implementation of TCM.

This course will provide an overview of the common physical and behavioral health comorbidities associated with BH I/DD Tailored Plan populations. Examples of interventions for screening, assessment, and management of these co-occurring disorders will be briefly presented along with self-management and self-help recovery resources (including substance use recovery). Participants will learn the roles and responsibilities for providing Whole Health Care using SBIRT, an evidenced-based model that provides Screening, Brief Interventions, and Referral for Treatment.

Objectives

As part of the TCM Continuing Professional Development series, participants will learn:

check Identify the common behavioral health and substance use problems in primary care.
check Discuss examples of brief interventions that can be used in primary care.
check Be able to apply the SBIRT model and discuss when to refer or hand-over to another provider.
check List the evidenced based screening tools available for common behavioral health and substance use problems that are frequently comorbid with medical problems.
check List options for patients to learn self-care and how to seek self-help support if needed.

Cancellation Policy

MAHEC has a pay-up-front policy for all CE programs. The only exceptions will be for pre-approved programs where an individual payment plan is appropriate. Registrations received without accompanying payment will not be processed and participants who have not paid the course fee will not be admitted into the program.

Unless otherwise noted in course materials, the following cancellation policy applies to all programs:

  • Cancellations must be in writing (fax, email, or mail).
  • Cancellations received more than 2 weeks prior to the event will receive 100% refund.
  • Cancellations received between two weeks and two full business days prior to the first day of the event are refunded at 70% of the registration fee subject to a minimum $25 cancellation fee.
  • No refunds or credits will be given for cancellations received less than two full business days prior to the event.
  • No vouchers will be issued in lieu of a refund.
