Tailored Care Management: Integrated Care Best Practices for Advanced Medical Home/Care Management Agencies

Description

Many care managers have limited experience working with healthcare providers. Likewise, many medical providers have limited experience working with care managers. This course aims to close this divide and present best practices in engagement, health, and behavioral health coordination. We will cover the tools and skills needed when working on health outcomes, working in teams, communication between providers, and how to best support the patient/client's medical and behavioral health goals.

Physicians, advanced practice providers, behavioral health care managers, nurse care managers, clinical supervisors, and care extenders (community health workers, peer support specialists, and qualified professionals)
