Healthy Opportunities Pilot Training for PIHPs and Tailored Care Managers Part 2 – This is the second in a series of Healthy Opportunities Pilot Trainings for Care Managers who provide Tailored Care Management to HOP Members. This multi-part course is comprised of 6 independent modules. Part 2 dives into each of the Pilot domains: Food, Housing, Transportation, Toxic Stress and Interpersonal Violence. The training provides guidance for Care Managers on the 29 services within these domains and the cross domain services. Through the training Care Managers will become familiar with the details of each service and service-specific eligibility requirements.

* How Care Managers Can Choose Appropriate Transportation Services, Version 2
* How Care Managers Can Choose Appropriate Health Related Legal Supports
* How Care Managers Can Choose Appropriate Food Services within the Healthy Opportunities Pilot Programs
* Understanding the Medical Respite Cross Domain Service
* How Care Managers Can Choose Appropriate Toxic Stress Services

The Framework:
North Carolina's Healthy Opportunities Pilot program is an unprecedented opportunity to test the integration of evidence-based, non-medical interventions into the state's Medicaid program. The Pilots will provide non-medical services to qualifying Medicaid members across four domains: housing, food, transportation and interpersonal violence/toxic stress. Critically, frontline care managers working in Pilot regions will play an essential role in identifying members that may benefit from Pilot services, recommending appropriate services and coordinating their care.
