TCM Home and Community Based Services (HCBS) Monitoring Training

Description

NC DHHS has developed partnerships with North Carolina Area Health Education Center (NC AHEC) to provide practice support, technical assistance, and continuing professional development to help Tailored Care Management providers who have applied and passed the desk review in the transition to Tailored Care Management. The TCM Continuing Professional Development programs are designed to assist TCM care managers, supervisors and extenders with evidenced-based practices in all treatment settings for the successful implementation of TCM.

This live webinar training will provide an overview of the assessment and monitoring of home and community-based services (HCBS). An overview of the services, responsibilities and timelines for HCBS will be provided to assist in the successful implementation of TCM to ensure appropriate monitoring and supervision of HCBS is provided to Medicaid members.

Target Audience

This course is for TCM certified AMH+/CMA providers and Tailored Plans.

CINs and other agencies providing comprehensive care management that do not have a contract with MAHEC do not receive these courses for free. The contact person in order to gain access to TCM educational materials is Scott Melton.

• Explain the purpose for home and community based services assessment and monitoring
• Identify various types home and community based service (HCBS)
• Utilize resources and assessments tools to monitor HCBS
• Identify timelines and circumstances triggering HCBS assessment and monitoring needs for Medicaid members
