In this module, you will learn about the Care Management for High-Risk Pregnancies (CMHRP) program and how it supports expectant mothers in North Carolina. You will explore the program's desired impact, including key health issues related to infant mortality that CMHRP seeks to address. You will also learn about eligibility criteria, the enrollment process, and the core components of CMHRP services. The training covers the role of the CMHRP Care Manager in coordinating care and collaborating with maternity providers, and highlights how you, as a CHW, can support members by connecting them to resources and helping them navigate the program.

NC AHEC, as an anchor partner in the NC CHW Initiative, is committed to ensuring that Community Health Workers have access to ongoing training and professional development. As a vital workforce advancing health outcomes across the state, CHWs must have the opportunity to deepen their knowledge and strengthen their skills to support the health and wellness of all North Carolinians.

With funding from the NC Department of Health Benefits (Medicaid), NC AHEC has collaborated with CHWs, CHW supervisors, and other key stakeholders to develop CHW Specialty Training standards, identify core training topics, and create a series of online modules designed to prepare CHWs to work alongside primary care practices within the Medicaid Managed Care network.

Each module offers continuing education credit recognized by the NC CHW Association for certification maintenance. Some modules address standalone topics, while others may be taken together to build deeper understanding in key areas. All modules are co-developed with CHWs to ensure the learning content and format reflect CHW culture and real-world experience.
