Participants will learn how to apply person-centered practices to assist members in various levels of care transitions through their lifespans. This course will cover care management skills and transitional care management best practices including needs assessments and care planning. This overview will also cover transitions to and from psychiatric hospitalizations, PRTFs, LTSS, the I/DD transition to adulthood, and adult to geriatric care and end-of-life care.
