Hospice and/or Palliative Care decisions with patients and their families can be challenging for providers. How do you start that conversation? Join us as we discuss tools and techniques in a clear and compassionate way. Topics will include clinical eligibility guidelines for each service regarding patients, their families and providers. In the continually changing world of healthcare, these services will become more important to our community as a stronger focus is placed on providing quality care for persons who are chronically, seriously or terminally ill outside of hospital walls.
