Attention: Due to Medicaid budget cuts, registration has been closed. NC DHHS DHB will hosting all future events of the TCM Learning Collaborative and the DHB Office Hours. Please contact the Department for additional information, links and dates of new events.

July 1st, 2021, marked the start of NC Medicaid Transformation with the transition to Standard Care Plans. NC DHHS began Tailored Care Management (TCM) on December 1st, 2022. The Department launched Tailored Plans on July 1st, 2024. During these implementations, DHHS developed a partnership with the North Carolina Area Health Education Center (NC AHEC) to provide practice support, technical assistance, and continuing professional development to help Tailored Care Management providers who have applied and passed the desk review in the transition to Tailored Care Management.

The TCM Learning Collaborative is designed to assist TCM providers in developing the administrative tools to implement best practices within all treatment settings and among all TCM providers. These sessions will be hosted by Continuing Professional Development Department at MAHEC to facilitate a coordinated learning experience with access to subject matter experts across the state. The TCM Learning Collaborative facilitates educational content on the second Wednesday of each month from 12:30 to 1:30 pm.

The TCM Learning Collaborative and DHB Office Hours will no longer be one event but will be separate programs to focus the Learning Collaborative on Care Managers and Extenders. The TCM Learning Collaborative consists of 1 one-hour live presentation focusing on best practices for TCM care management, designed for Care Managers, Supervisors, and Extenders. These sessions carry continuing professional education credit.

Please note: All those who register will be added to the TCM email list for announcements from NC DHHS DHB and NC AHEC TCM notifications. You must register each year to continue receiving notifications on TCM updates from NC AHEC and the Department. DHB Office Hour sessions will be recorded and posted for future access. By attending this event, participants agree to the audio and video recording of the session, as well as the posting of the recordings in an LMS for TCM providers to access.

Audience

Administrators, TCM care managers, extenders and supervisors from TCM certified advanced medical home+ (AMH+) and care management agencies (CMA) providers including physicians, nurses and various behavioral health substance use professionals

Objectives

Participants will:

check Stay updated on TCM policies and procedures from NC DHHS DHB
check Engage in conversation with NC DHHS and provide valuable feedback on the administration of TCM in NC
check Identify various health conditions and care management techniques for chronic disease states for coordinating TCM care management
check Identify strategies to implement best practices in TCM
check Make recommendations to the Department to improve TCM services

Cancellation Policy

MAHEC has a pay-up-front policy for all CE programs. The only exceptions will be for pre-approved programs where an individual payment plan is appropriate. Registrations received without accompanying payment will not be processed and participants who have not paid the course fee will not be admitted into the program.

Unless otherwise noted in course materials, the following cancellation policy applies to all programs:

  • Cancellations must be in writing (fax, email, or mail).
  • Cancellations received more than two weeks prior to the event will receive 100% refund.
  • Cancellations received between two weeks and five full business days prior to the first day of the event are refunded at 70% of the registration fee subject to a minimum $25 cancellation fee.
  • No refunds or credits will be given for cancellations received less than five full business days prior to the event.
  • No vouchers will be issued in lieu of a refund.
  • Transfers/substitute(s) are welcome (please notify us in advance of the program).
