Healthy Opportunities Pilot Statewide Conference: HOP - Embracing Success and Exploring What the Future Holds

The HOP Statewide Conference on August 7th aims to convene all stakeholders participating in all three Pilot regions. It is an opportunity to bring together all collaborating partners to celebrate Pilot successes, share experiences and lessons learned, identify existing challenges and plan actionable solutions, facilitate continuous collaboration to break down silos, and share preliminary Pilot evaluation findings. The day's agenda will feature a plenary session, evaluation presentation, breakout sessions, and networking opportunities. This conference will be provided at three in-person hub sites, one in each Pilot region, with the option to join virtually.

What to Expect

Plenary Session
The plenary session will feature two leaders, one from NC DHHS and one from Human Service Organizations. The NC DHHS Leadership will welcome attendees and deliver remarks on the role of the Pilots in Medicaid Transformation and the future of Medicaid in North Carolina, and the impact this has on the health and well-being of North Carolinians. The HSO Leadership will deliver remarks on the role of the Pilots in expanding organizational capacity of Human Service organizations, their experience delivering services, and the impact participating in the Pilot has had on their community.


Evaluation Session
Pilot evaluators from NC DHHS, The Cecil G. Sheps Center For Health Services Research at UNC Chapel Hill, and the Duke Margolis Center for Health Policy will present on evaluation efforts, including an overview of methodology and preliminary findings. Presentations will be followed by a panel Question and Answer session.


Breakout Sessions
Afternoon breakouts will feature tracks or concentrations. All participants may register to attend one track. Please note, if you have an interest in multiple tracks, all tracks and sessions will be available to view for 90 days after the conference through the event platform app- EventMobi.

Track One- HOP at the Policy level: This track focuses on policy opportunities to strengthen and expand the Pilot. Topics for presentation may include:
• Policy innovations to facilitate enrollment and referrals such as revising eligibility criteria
• Strategies for Pilot expansion
• Building HSO capacity and sustainability, for example through payment models
• Including equity-based principles into Pilot structure at the policy level
• Strategies to increase Pilot resources in low-resource areas

Track Two: HOP at the Implementation level/Practice level: This track focuses on best practices and building strategy among Pilot partners involved in implementing the Pilot. Topics for presentation may include:
• Best practices for utilizing technical supports such as NCCARE360
• Strategies for training and increasing Pilot knowledge among Care Managers, and HSO staff
• Best practices managing the enrollment and referral processes
• Strategies for integrating Pilot processes into daily workflows for Health Plans, Care Managers, and HSO staff
• Including equity-based practices in Pilot processes
• Best practices for managing service delivery in the interpersonal violence domain

Track Three: HOP Strengthening Communication and Partnership: This track focuses on growing partnerships and strengthening communication to foster trusting networks. Topics for presentation may include:
• Strategies to increase Pilot awareness, such as through targeted messaging or social media
• Strategies for effective communication internally and externally
• Identifying and including new partners – for example Community Health Workers in the Pilot
• Best practices for engaging Members and improving services based on their experiences


Framework: North Carolina's Healthy Opportunities Pilot program is an unprecedented opportunity to test the integration of evidence-based, non-medical interventions into the state's Medicaid program. The Pilots will provide non-medical services to qualifying Medicaid members across four domains: housing, food, transportation and interpersonal violence/toxic stress.
