Primary Care and Specialists are experiencing growing pressure to perform well on value-based payer contracts. With the transition to managed Medicaid in the state of North Carolina, these pressures have increased. Practices have become more familiar with quality performance improvement, but few practices (less than 10%) have experience using utilization and cost data to improve care delivery. Participants in this series will be provided with clear examples of how claims and cost data can support achieving the quadruple aim.

The goal of this ECHO series is to support practices and providers in achieving high performance under the new value-based payment models. The course will focus on strategies that improve practice performance by providing education and updates on clinical process improvement, specialist co-management, cost of care drivers, strategies to impact readmissions, and ED utilization. WNC Specialists and PCPs will present cases and engage in conversation around efforts to improve care integration, quality performance, and cost of care.

Project ECHO® is a lifelong learning and guided practice model that revolutionizes medical education to provide best-practice specialty care and reduce health disparities. ECHO is led by an expert team that uses multi-point videoconferencing to conduct virtual learning sessions comprised of a short didactic followed by case discussions presented by attendees. This is the participant's chance to discuss difficult patient cases with a panel of experts and colleagues to gain feedback. When registering for this Project ECHO®, please be prepared to share a case discussion during one of our sessions.

This video teleconferencing series, guided by experts, takes place over six modules (1-hour each) and covers topics such as:
• Improving Population Health through Process Improvement
• Using Data to Impact Affordability - Case Reviews in Hematology
• Impacting Readmissions and ED Utilization: A Focus on Respiratory Illness
• Improving Outcomes through Transitions: Collaboration with Post-Acute Care and Home Health
• Behavioral Health Co-Management
• Process Improvement for Pediatric Measures
