The NC DHHS Division of Health Benefits (DHB) has partnered with NC AHEC to assist in the implementation of Tailored Care Management, which was launched December 1st, 2022. NC AHEC provides practice support, technical assistance, and continuing professional development to help Tailored Care Management providers who have applied and passed the desk review in the transition to, and continuous quality improvement of, Tailored Care Management. The TCM Learning Collaborative is designed to assist TCM providers in developing the administrative tools to implement best practices within all treatment settings and among all providers. The Collaborative will be led by NC AHEC Practice Support to facilitate a coordinated learning experience with access to subject matter experts across the state.

This course is not intended for sale. It is for certified AMH+/CMA providers and Tailored Plans only. A discount code to allow free access will be provided to you by your agency administrator. CINs and other agencies providing comprehensive care management that do not have a contract with MAHEC do not receive these courses for free. To contract with MAHEC, please contact Scott Melton.


This session will cover required responsibilities for office-based and hospital emergency services, as well as walk-ins for TCM members. Requirements, structure, and a short overview of transitional services and reporting requirements will be discussed.

Target Audience

TCM care managers, supervisors, and extenders from the Tailored Plans, TCM certified advanced medical homes+ (AMH+) and care management agencies (CMA)

  • Describe the crisis requirements for TCM certification
  • Identify best practices from established TCM agenices
  • Start developing operational policies and procedures to implement crisis walk-in services for TCM certification
