This comprehensive online course, developed by the North Carolina Department of Health and Human Services (NC DHHS), Division of Health Benefits (DHB), equips Tailored Care Management (TCM) care managers with essential knowledge and practical strategies to support members living with complex chronic health conditions. The trainings emphasizes person-centered care, preventive screenings, early intervention, care coordination, and health education, with a focus on managing high-impact chronic illnesses including diabetes, heart failure, cardiovascular and renal conditions, pulmonary diseases, and co-occurring conditions in individuals with behavioral health and intellectual and developmental disabilities (I/DD). This course is designed to improve chronic disease health competency for TCM care managers and extenders to improve the overall health outcomes of Medicaid members and reduce health disparities among TCM populations.

This bundle contains:

  • Diabetes (0.75 hours)

  • Heart Failure (0.75 hours)

  • Hypertension, Hyperlipidemia, and Chronic Kidney Disease (CKD) (0.75 hours)

  • The Fatal Five for People with I/DD (1.25 hours)

  • Chronic Obstructive Pulmonary Disease (COPD) and Pulmonary Hypertension (0.5 hours)

  • Preventive Screenings (0.75 hours)


This course meets the criteria for 4.75 hours towards the TCM Annual Refresher trainings. Please note that there are two ways to receive educational credit for the Care Management for Members with Chronic Health Conditions series: participants must attest to attending these trainings in the live TCM Learning Collaborative setting or watching the content in this bundle in its entirety. Licensing boards will not accept both the Learning Collaborative Year 4 credits and this course as credit towards licensure.

Participants have unlimited time to complete this course and access course materials.

CINs, Standard Plans and other agencies providing comprehensive care management that do not have a contract with MAHEC do not receive these courses for free. Audience

Psychologist, Licensed Mental Health and Substance Use Counselors; TCM Care Managers, Supervisors, and Extenders from the Tailored Plans; Standard Plans and TCM Certified Advanced Medical Homes+ (AMH+), and Care Management Agencies (CMA).

Objectives

Participants will be able to:

check Identify key clinical considerations and care strategies for managing diabetes in TCM populations
check Recognize risk factors and management approaches for hypertension, hyperlipidemia, and chronic kidney disease, and understand their interrelated impacts on health
check Explain the role and recommended frequency of preventive health screenings in the management of chronic conditions across lifespan populations
check Describe evidence-based interventions for managing heart failure and promoting medication adherence and symptom monitoring
check Apply care coordination practices for members with COPD and pulmonary hypertension to reduce hospitalizations and improve quality of life
check Demonstrate an understanding of the "Fatal Five" health risks affecting individuals with IDD and incorporate preventive and responsive strategies in care planning

Cancellation Policy

MAHEC has a pay-up-front policy for all CE programs. The only exceptions will be for pre-approved programs where an individual payment plan is appropriate. Registrations received without accompanying payment will not be processed and participants who have not paid the course fee will not be admitted into the program.

Unless otherwise noted in course materials, the following cancellation policy applies to all programs:

  • Cancellations must be in writing (fax, email, or mail).
  • Cancellations received more than two weeks prior to the event will receive 100% refund.
  • Cancellations received between two weeks and five full business days prior to the first day of the event are refunded at 70% of the registration fee subject to a minimum $25 cancellation fee.
  • No refunds or credits will be given for cancellations received less than five full business days prior to the event.
  • No vouchers will be issued in lieu of a refund.
