Participants will be able to:

Recognize risk factors and management approaches for hypertension, hyperlipidemia, and chronic kidney disease, and understand their interrelated impacts on health |

Explain the role and recommended frequency of preventive health screenings in the management of chronic conditions across lifespan populations |

Describe evidence-based interventions for managing heart failure and promoting medication adherence and symptom monitoring |

Apply care coordination practices for members with COPD and pulmonary hypertension to reduce hospitalizations and improve quality of life |
|
|
Demonstrate an understanding of the "Fatal Five" health risks affecting individuals with IDD and incorporate preventive and responsive strategies in care planning |

This fee selection is for use with a discount code. It is only for certified AMH+/CMA TCM providers who have passed readiness review with NCQA. Sharing the discount code or TCM Web Portal with an unapproved agency can result in either agency being billed the stated amount for this course. The Mountain Area Health Education Center designates this entire online continuing education activity as meeting the criteria for .50 CEUs as established by the National Task Force on the Continuing Education Unit.
