NC DHHS has developed partnerships with North Carolina Area Health Education Centers (NC AHEC) to provide practice support, technical assistance, and continuing professional development to help Tailored Care Management providers who have applied and passed the desk review in the transition to Tailored Care Management. The TCM Continuing Professional Development programs are designed to assist TCM care managers, supervisors, and extenders with evidence-based practices in all treatment settings for the successful implementation of TCM.

People with disabilities and their families often navigate multiple systems of care to address basic and complex healthcare needs. Supported decision-making (SDM) offers a person-centered approach that places the individual at the center of care and decision-making. This session will provide an overview of SDM models, tools, best practices, and applications in healthcare and community life. Participants will have the opportunity to review examples of how SDM enables individuals to make informed choices that promote quality of life, self-determination, and optimal health.

TCM care managers, supervisors, and extenders from the Tailored Plans, TCM certified clinically integrated networks (CINs), TCM certified advanced medical homes+ (AMH+) and care management agencies (CMA)

Objectives

The National Academy of Certified Care Managers (NACCM) certifies that continuing education training provided by a NC Area Health Education Center shall be approved for no more than 15 contact hours for any given Care Manager Certified (CMC) recertification period. Cancellation Policy

MAHEC has a pay-up-front policy for all CE programs. The only exceptions will be for pre-approved programs where an individual payment plan is appropriate. Registrations received without accompanying payment will not be processed and participants who have not paid the course fee will not be admitted into the program.

Unless otherwise noted in course materials, the following cancellation policy applies to all programs:

  • Cancellations must be in writing (fax, email, or mail).
  • Cancellations received more than two weeks prior to the event will receive 100% refund.
  • Cancellations received between two weeks and five full business days prior to the first day of the event are refunded at 70% of the registration fee subject to a minimum $25 cancellation fee.
  • No refunds or credits will be given for cancellations received less than five full business days prior to the event.
  • No vouchers will be issued in lieu of a refund.
  • Transfers/substitute(s) are welcome (please notify us in advance of the program).
