NC DHHS has developed partnerships with North Carolina Area Health Education Center (NC AHEC) to provide practice support, technical assistance, and continuing professional development to help Tailored Care Management providers who have applied and passed the desk review in the transition to Tailored Care Management. The TCM Continuing Professional Development programs are designed to assist TCM care managers, supervisors and extenders with evidence-based practices in all treatment settings for the successful implementation of TCM.

Description: This course dives into the crucial aspects of medication management, focusing on reconciliation, monitoring, and patient adherence within the context of Tailored Care Management (TCM). Participants will gain practical skills and knowledge essential for optimizing patient care, ensuring medication safety, and improving health outcomes. Participants will explore effective strategies for reconciling medications during key transitions, best practices for provider medication monitoring to improve patient adherence and health outcomes.
This course is not intended for sale. It is for certified AMH+/CMA providers and Tailored Plans only. A discount code to allow free access will be provided to you by your agency administrator. Target Audience
TCM certified advanced medical homes (AMH), care management agencies (CMA), and LME/MCO administrators, prescribers, pharmacists, nurse care managers, TCM care managers, clinical supervisors, and care extenders.
Objectives
Participants will be able to:
- Determine the importance of medication reconciliation in healthcare settings and its role in preventing adverse drug events
- Identify systematic approaches and best practices for conducting medication reconciliation across various healthcare settings and transitions
- Explore roles and methods for monitoring medications, including assessing effectiveness, identifying side effects, managing drug interactions, and improving communication to the care team
- Gain insights into patient-centered strategies for promoting medication adherence and addressing barriers
- Develop patient education and communication skills to facilitate collaborative decision-making with patients, caregivers, and healthcare providers regarding medications
- Acquire best practices for implementing safe and effective medication reconciliation, medication monitoring and adherence protocols within clinical practice settings
