Vital Signs: Real Food. Real Medicine. Real Change. is a three-day, in-person continuing medical education conference designed to equip healthcare professionals with practical, evidence-based strategies to implement Food as Medicine interventions in clinical, corporate, and population health settings.

Despite growing evidence supporting plant-forward, whole-food dietary patterns for the prevention and treatment of chronic disease, many clinicians lack the confidence, tools, and systems-level pathways to translate nutrition science into routine practice. This activity addresses that gap by moving beyond theory to focus on actionable clinical workflows, counseling tools, referral pathways, and scalable implementation models.

Through expert-led lectures, case-based discussions, applied workshops, and real-world implementation examples, participants will learn how to prescribe structured food-based interventions, integrate nutrition and coaching pathways into care models, and evaluate outcomes across cardiovascular disease, type 2 diabetes, obesity, hypertension, and inflammatory conditions.
