About this Module

In this module, you'll learn how to define prediabetes and diabetes and identify their prevalence and risk factors. You'll learn how to recognize key clinical values that are important for people living with these conditions and explore the most common prevention and intervention strategies, including healthy diet, physical activity, stress management, and sleep habits. You'll also learn how Community Health Workers can support individuals with diabetes in their self-management and recognize when urgent or emergency care may be needed.

Approximate time to complete the course: 2.50 hours

Learner content includes: Video, corresponding course handout, exercises and a brief post test.

NC AHEC, as an anchor partner in the NC CHW Initiative, is committed to ensuring that Community Health Workers have access to ongoing training and professional development. As a vital workforce advancing health outcomes across the state, CHWs must have the opportunity to deepen their knowledge and strengthen their skills to support the health and wellness of all North Carolinians.

With funding from the NC Department of Health Benefits (Medicaid), NC AHEC has collaborated with CHWs, CHW supervisors, and other key stakeholders to develop CHW Specialty Training standards, identify core training topics, and create a series of online modules designed to prepare CHWs to work alongside primary care practices within the Medicaid Managed Care network.

Each module offers continuing education credit recognized by the NC CHW Association for certification maintenance. Some modules address standalone topics, while others may be taken together to build deeper understanding in key areas. All modules are co-developed with CHWs to ensure the learning content and format reflect CHW culture and real-world experience.

Community health workers, lay health workers, outreach workers, and other frontline public health workers improving the health of their communities, and the supervisors, colleagues and allies of the CHW workforce

Cancellation Policy

MAHEC has a pay-up-front policy for all CE programs. The only exceptions will be for pre-approved programs where an individual payment plan is appropriate. Registrations received without accompanying payment will not be processed and participants who have not paid the course fee will not be admitted into the program.

Unless otherwise noted in course materials, the following cancellation policy applies to all programs:

  • Cancellations must be in writing (fax, email, or mail).
  • Cancellations received more than 2 weeks prior to the event will receive 100% refund.
  • Cancellations received between two weeks and two full business days prior to the first day of the event are refunded at 70% of the registration fee subject to a minimum $25 cancellation fee.
  • No refunds or credits will be given for cancellations received less than two full business days prior to the event.
  • No vouchers will be issued in lieu of a refund.
  • Transfers/substitute(s) are welcome (please notify us in advance of the program).
