Welcome to the Western North Carolina region Triple P Provider page! Here you will find all necessary forms and tools for quarterly submission of Triple P data.
PASS: Peer Assisted Supervision and Support
Triple P Providers participate in Peer Support to allow opportunity for ongoing coaching in order to refine their strengths and improve their program delivery. Click the link below for key steps on hosting a Triple P Peer Support Meeting.
The PASS agenda is included below to help facilitate Peer Support meetings.
Data Submission Tools
Please use the Data Submission Template to compile and submit data to the Triple P Data Evaluator on a quarterly basis. Do not submit any information that includes personal identifiers for your clients. You may contact us with any questions you have about this process using the phone numbers and email addresses below.
Evaluation tools include forms or instruments that are used to collect data for use in ongoing program evaluation. Triple P uses specific tools that have been developed for each level of intervention. Please use the appropriate forms provided here to gather information for the Triple P levels you provide. Please do not send copies of the Evaluation Tools to us. Instead, complete the appropriate sections of the Data Submission Template using the information you have gathered, and submit only the Template to us on a quarterly basis.
